Rule 3.1000. Format of supplemental and further discovery

(a) Supplemental interrogatories and responses, etc. In each set of supplemental interrogatories, supplemental responses to interrogatories, amended answers to interrogatories, and further responses to interrogatories, inspection demands, and admission requests, the following must appear in the first paragraph immediately below the title of the case:

(1) The identity of the propounding, demanding, or requesting party;

(2) The identity of the responding party;

(3) The set number being propounded or responded to; and

(4) The nature of the paper.

(b) Identification of responses Each supplemental or further response and each amended answer must be identified by the same number or letter and be in the same sequence as the corresponding interrogatory, inspection demand, or admission request, but the text of the interrogatory, demand, or request need not be repeated.
